Deku is such a sweetheart in cannon that even as a villain, he'd be good at comforting his partner.

I assume you'd been feeling dysphoric for a while, which he would most definitely pick up on. Even if it's just little things like looking in the mirror longer, off handed comments, or you stop actually getting dressed in clothes other than sweats and oversized goodies.

So he'd drop small comments like: “There's my pretty boy”, “You're so so handsome baby”, “Darling, you're such a handsome boy”

When (and if) you finally break down, Izuku would be there for you completely. If Shigaraki wanted Izuku to go on a mission when that happened, Izuku would throw a knife at his head.

He'd cuddle you and mutter so many praises and gender affirming statements you'd feel so much better by the time dinner was ready.
