Yeah, that’s a real image Naoko drew. It is supplementary which means that you could, if you really wanted, consider it “non-canon”, but as far as a narrative, it shows Naoko’s intentions.
My posts about sexuality in SM are largely based on the manga (and Crystal), not the 90s anime. Where I mentioned anime before I was referring to all anime/manga in general. 90s Anime downplayed a lot of those elements. That one episode of S where Makoto gets a crush on Haruka is a pretty good example of both how the anime changed Usagi’s attitude towards sexuality, and how Japan definitely sees “girl crushes” as gay. All the girls flip out over Makoto’s crush because they’re afraid that she might be gay. It’s kinda crummy as far as a narrative goes but it does illustrate that the characters (and audience) acknowledge Makoto’s interest in Haruka might not be platonic. Usagi also teases Rei about owning takarazuka revue merchandise (all-female acting troupes) and relating it to the plot with Makoto & Haruka, implying she must be a little gay to like something like that; both of them show Japan’s understanding that these behaviours are not accepted as 100% straight, while also illustrating the implication that pursuing these interests into anything serious would change them from harmless to wrong.I think comparing the 90s anime to the manga is a pretty good illustration of how Usagi & company’s behaviour in the manga is not seen as straight in Japan, since the 90s anime kind of did go “no homo” on her. For example, in the manga, Usagi was flattered by Haruka’s attention to her and actually questioned her own sexuality. In the anime, Usagi freaked out and reacted very negatively to any of Haruka’s flirting.I guess if you’ve only seen the anime, and you hear people talking about scenes from the manga/Crystal/PGSM, I could understand they would think there was some misunderstanding, since the anime treated those scenes a little differently.
That being said, being in a straight relationship does not at all mean that Usagi cannot be attracted to other genders.

I think anon’s pointing out Usagi’s hetero attraction to point out that regardless of any fawning over other girls, she’s still in a straight relationship so we shouldn’t give the show too much credit. :/
Isn’t that basically saying that bisexual people stop being queer/become straight if they’re in a straight relationship, though…? Sailor Moon deserves the credit it gets.
