Himaruya might hint in Chapter 428 that Portugal and Spain are twins.
The answer to the first question is November 25, 2006. “1125” in Japanese could be read as “iihutago”, meaning “good twins”. So on this day, in Twitter and Pixiv, many Japanese artists draw fan arts of twins. Himaruya might choose this date as the correct answer to hint that Portugal and Spain are twins.
Portugal says that the answer to the first question is about him and Spain, and Spain also says that it is he and Portugal’s important day. In this case, “good twins” day becomes Portugal and Spain’s important day to celebrate their being twins.
In fact, the day of the cooperation agreement of tourism is not well known for the rest of the world, and among the list of dates given by Spain here after, this date (in the year of 2006) is either the earliest or the latest. Did Himaruya choose this date as the answer from this list randomly? I believe that he indeed hints something.
In the official book, Collezione, published last year, there is no mention of Portugal and Spain being brothers, but still it does not mean that they are not brothers. The fun of Portugal and Spain is that every reader can tell they look like identical twins, but they themselves deny it. So if Himaruya confirms that they are twins, it would spoil the fun. And the official confirmation is not so necessary since everyone can tell from their appearance that they are twins.
On the other hand, Canada and USA are confirmed “adopted brothers” in Collezione, which is the case that their blood relation is officially denied.
Another case is that China and other Chinese districts are commonly believed by fans that they are related by blood, but this relationship is also not mentioned in Collezione.
Some fans might be offended by the hinted incest of Portugal and Spain. But this is not the first time of hinted incest in Hetalia. In Hetalia anime, Belarus asks her brother, Russia, to marry her. Also in the Hetalia movie, Paint it White, Switzerland and Lichtenstein enjoy themselves together as if having a honeymoon, while the rest of the world is in chaos. Even in fandom, Prussia and Germany is a popular pairing. And as for UK and USA, although they are not blood-related, it is still awkward enough that adopted brothers end up in lovers.

“’Mando, ‘mando!” He could hear his little brother calling out to him, but young Armando just wanted to play outside with his friends. Not spend his time with Antonio, who had started to follow him around everywhere since the kid had learned to walk on his own. Sort of.
Then their mother had read that fairytale to them and Armando had assisted her. Since that one time ‘Tonio wanted him to tell him the story all the time, because only Armando did the voices just right.
And as soon as the story ended, his baby brother would demand that the story would be told again from the very beginning.
He couldn’t wait for the kid to learn how to read himself and leave him be, but for now he had to find a hiding place where Antonio wouldn’t be able to find him or he would have to read him the same book for the fifth time today.

Antonio stumbled inside, having finally made it all the way home after a long day together with his best friends. He loved it when he, Francis and Gilbert managed to spend time with one another. Still, the Spaniard was happy to be home again.
His plan was to get changed into something comfortable and clean, none of their clothes ever seemed to stay clean when they did anything together. Which really had to be killing Gilbert even if he would play it cool and never admit to it, Germany had to get it from someone.
He also desired a nice glass of wine, which he wanted to drink sitting in his back garden looking at the stunning view. So he got changed, grabbed a bottle and a glass and was about to walk into his garden when he noticed the door wasn’t closed. Tony was sure that he had closed and even locked that door.
He set the bottle and glass aside and cautiously walked outside, but not careful enough because as soon as he was fully out of his house a voice called out his name.
It was a voice he knew all too well and then he saw someone sitting in one of his garden chairs. On the table was another bottle of wine and two filled glasses.
There was his brother, the one he was never sure he got along with or not, waiting for him to get home and spend the evening looking at the view.

All of this was Antonio’s fault, he decided, because everything that went wrong in this messed up family unit was always that man’s fault. Even the fact they were a dysfunctional family, and not even all related to each other, was Antonio’s fault.
Calling this so named Tomato gang together was also the Spaniard’s fault. Why did he keep dragging Johan back into this illusion of a family? The next question the Dutchman asked himself, had to ask himself, was why he allowed himself to be dragged into these informal meetings. He fought for decades to get the freedom he had now. Freedom for himself and his people. Freedom from Spain and from this messed up family that that man had forged.
None of his bosses would have forced him to attend this informal meeting if he had insisted on being too busy to go on a vacation to Spain. If only he had told them he had too much paperwork to handle right now, needed to catch up on some extra sleep or simply said to that he had no desire to go meet with the other nations. They would not have forced him to go, rather have him absent from some small get together than start an international incident and he could have stayed comfortable at home.
But that last one would have been a lie. There were people there that Johan did want to see. One of them was Emma and he wanted to see his sister more often but even though their homes were so close to each other they were both so busy that they didn’t get together as often as he wanted them to. Then again, Johan would like to see his both of his sibling every day.
So here he was, at a party he didn’t want to be at and in his room to avoid all the whispers about how nice it was that both Johan and Rosália were there. Not willing to fuel their gossip he stayed away, but Holland was happy to see Portugal.
“I don’t like her like that.” He told himself as he walked circles in his bedroom. There had been the years that he had had this crush on her, but that was back when he had still been in his teenage years and Portugal had been the first one to take control of the seas. He was and always had been at the water’s mercy and so he had been awed at her skill on board of a ship.
“Me? Liking her” No, there was no way he still felt like that. That crush was from so long ago and from a boy, not a man. He no longer held these feelings for her.
“That’s… no...” He said aloud. He was not in love, he was not, and he would show all of them he did not love her by enjoying this evening without even glancing her way.
He left his room with full confidence of his plan but after no more than four or five steps he knew this plan was doomed to fail because he wouldn’t be able not to look at her. The truth was that he truly loved Roasália, it had never simply been a crush. No, Johan had been in love with her from the first time he has seen her in charge of a ship.
