I felt the need to ask my friend

A rough drawing of an elderly unicorn, Shining Armor, resting his head on some giant pinkish yellow thing curled around him. He asks in a weak voice "Will you love me? Though I am a worm?"

The large curled object lifts away, as an ethereal and booming voice says "Oh, my shining light, my armor in the dark"

A huge deer-like nose, many times bigger than shining armor, comes into frame. The pink and purple colors reveal it to be the end of Cadance's muzzle.
Her great voice softly booms "I am a worm too"
"No," Shining armor says, planting a kiss on her nose. "You are a butterfly, transformed beyond what I could be"

A great eye looms behind Shining Armor, almost engulfing him. Cadance's words are heavy with welling tears. "I didn't ask for this"
"I know," Shining armor murmurs. "I didn't ask for you, either. And yet..."

"I don't deserve you," she frets, eyelashes curling over his back
"You deserve much more than me"
"I don't want it"

"Take it," he says, looking back at her over his shoulder. "Take it and forget us worms"

A young pegasus Cadance appears in a flashback, with no horn, and fairly normal horse proportions.
Cadance recalls her past self fondly. "I will never forget being a worm"
The memory moves forward, to when Cadance and Shining Armor met. She is an alicorn now, with a pink and ivory curled horn sprouting from her forehead, and hair beginning to fade to mist at the edges, like diaphanous split ends. She is looking down at shining armor, now a few feet below her and only a few degrees smaller. They were closer in size when they met, so many years ago.
She continues "I will never forget you."

"But you will live?" the ancient unicorn asks her, again in present day.
Tears are spilling from her glossy eyes and pooling around his knees.
"I will live"

He lays his head down, whispering "And I will love you, always"
Her eye slides closed, the lashes caressing him like a blanket.

The rest of Cadance's massive head lays in a small lake of her own making, tears spilling freely now. Shining Armor is so small compared to her, he almost disappears from view.
"I will love you," she says. "Because you are a worm"

Cadance's body could be a mountain range of soft pink feathers, resting on the ground and caressing her love with the lashes of a single eyelid.
The form he rested in before is revealed to be her horn, now lifted into the air and casting its shadow on the two of them, holding each other as close as they can, for as long as they can.
She says her last words to him as the lake spreads further and further.
"You are the world to me."
